Provides nursing care to patients by utilizing the nursing process and meeting established hospice standards. Accountable for assigned caseload in collaboration with other interdisciplinary team members. Serves as advocate for patient and families. One year of clinical experience in acute care setting or equivalent experience required (additional experience in hospice, homecare or oncology beneficial). Current RN license required along with basic computer skills, strong communication and interpersonal skills. Need to be CPR and mandatory reporter certified or willing to become certified.
Aide provides patient care and follows plan of care. Assigned territory will be Nebraska. Must have 75 hour Home Health Aide supervised training course or nurse aide training program. Six months experience as a Nurse Aide strongly preferred. Current CPR status required. Current driver’s license and auto insurance required along with available transportation to travel to patient residences. Must be able to meet physical requirements of the position.
Siouxland Palliative Care, a division of Hospice of Siouxland, has an opening for a Nurse Practitioner. The ARNP performs patient physical assessments and recommends and orders treatment interventions within the ARNP scope of practice. ARNP will assist families with navigating the health care system; foster seemless, rather than fragmented care, to maximize quality of life for patient and their family. ARNP may see patient in their home, a nursing facility, clinic, or other setting.
Must have master’s degree in nursing with completion of Iowa Board approved advanced practice master’s program or completion of a formal advanced practice education program (family, adult, or geriatric). Current NP certification required by a national professional certifying body recognized by the Iowa, Nebraska, or South Dakota Boards of Nursing. Current ARNP license required that’s issued by State of Iowa, Nebraska, and South Dakota Boards of Nursing. Current prescriptive authority through the State of Iowa Board of Pharmacy and current DEA registration required. Must be registered as a practitioner with the Federal Drug Enforcement Administration.
Preferred applicant will have a minimum of one year recent clinical nursing experience along with strong clinical skills in physical assessment for all ages. Must be able to function independently within parameters of nurse practitioner/ physician protocols in provision of caring for patients served by Siouxland Palliative Care. Effective organizational skills are needed as are effective oral and written communication skills. There will be frequent travel between sites and homes.
Full-time RN needed for Nebraska Team to provide nursing care to patients by utilizing the nursing process and meeting established hospice standards. Accountable for assigned caseload which would be South Sioux City, NE and into local South Dakota service area. Would be trained to cover our Nebraska service area too. Will work in collaboration with other interdisciplinary team members. Serves as advocate for patient and families. One year of clinical experience in acute care setting or equivalent experience required (additional experience in hospice, homecare or oncology beneficial).
Current RN license required along with basic computer skills, strong communication and interpersonal skills. Need to be CPR and mandatory reporter certified or willing to become certified.
Must be a registered nurse with current licensure from State Board of Nursing, BSN preferred. A minimum of one year professional nursing experience required. Community health nursing or oncology experience preferred. Coordinator works collaboratively and facilitates communication between team members, patients/families, the patient’s physician and other health care providers to assure coordination of services. Schedule includes two weekdays from 8 AM – 5 PM and every other weekend.
Typical Duties and Responsibilities:
Documents all patient care related activities accurately, legibly, and in compliance with hospice policies. Facilitates communication between team members and the patient’s physician to assure coordination of services and implementation of orders. Demonstrates knowledge of and complies with Hospice of Siouxland Safety and Infection Control policies and procedures. Makes clinical decisions independently based on standards of care. Uses effective communication principles to coordinate care with other health care providers, ie, physicians, primary nurse, lab, Hospice Aides, and therapies. Recognizes and communicates issues/occurrences utilizing established lines of authority, and assists in identifying opportunities for improvement when desired outcomes are not achieved. Actively participates in meetings, committees, conferences, and inservices. Assists with the orientation of new staff. Responsible for complete confidentiality of patient and caregiver records and any and all financial or operational confidential information. Schedules patient visits in EMR to assure all visits are scheduled according to visit frequency in patient’s Plan of Care. Assures customer service response to patients/families, physicians, and other health care providers meets expectations including promptness, responsiveness and follow through to meet customer needs. Services as a member of Quality Improvement Committee or other committees as requested. Effectively manages time in a constructive manner without direction to reach goals. Triages patient/family phone calls to coordinate visits/interventions with other staff assigned to provide care. Assists in taking Hospice of Siouxland and Palliative Care referral information and documenting in the EMR along with coordinating Intake visit.
Full-time position available. Overnight schedule would be approximately 7 PM to 7 AM; would be three 12-hour days, and every other weekend. Time off during the week if working the weekend. RN provides nursing care to patients following established hospice standards. Responds to all after hours phone calls for entire geographic area. Serves as an advocate for patients/families.
Must have current RN license in IA, NE, and SD as applicable. One year professional nursing experience strongly preferred. Current CPR certification and child and adult mandatory reporter certification required. Current driver’s license and auto insurance required along with available transportation to travel to patient residences.
Must be able to meet physical requirements of the position.
Hospice of Siouxland seeks licensed/certified therapists to provide the following complimentary therapies to patients of Hospice of Siouxland. These will be contracted positions in our tri-state service area. Therapist must have their own pet/equipment to provide therapy services.
