Provides nursing care to patients by utilizing the nursing process and meeting established hospice standards. Accountable for assigned caseload in collaboration with other interdisciplinary team members. Serves as advocate for patient and families.
One year of clinical experience in acute care setting or equivalent experience required (additional experience in hospice, homecare or oncology beneficial). Current RN license required along with basic computer skills, strong communication and interpersonal skills. Need to be CPR and mandatory reporter certified or willing to become certified.
Under limited supervision, provides spiritual care services which include identification of spiritual needs of patients and families and networking with community pastors to assure spiritual needs are being met. Provides spiritual counseling to patients and families respecting their belief system.
Bachelor’s degree in theology, counseling, psychology, or similar field required; master’s degree preferred. Two years of active ministry experience strongly preferred. Excellent interpersonal and counseling skills are required along with the ability to address spiritual care issues and concerns for all religious faiths. Minimum 2 units Clinical Pastoral Education preferred. Basic computer skills/knowledge required.
Makes nursing visits to patients for assigned team. Minimum of one year nursing experience strongly preferred. Current CPR status required. Must demonstrate excellent written and verbal communication skills along with strong computer skills.
Schedule would include weekend rotation.
Provides a variety of social work services for both Hospice of Siouxland and Siouxland Palliative Care which include patient/family counseling, networking of available resources, and aligning them with the appropriate services to provide a continuum of care. Requires a bachelor’s degree in social work, psychology or counseling or related experience. Master’s degree with major course work in social work, psychology, or counseling preferred. Clinical experience of skills relevant to grief and/or loss preferred. Must communicate effectively with all members of the health care team. Basic computer skills necessary.
Makes nursing visits to patients in entire geographic service area. Minimum of one year professional nursing experience strongly preferred; critical care nursing experience preferred (additional experience in hospice, homecare or oncology beneficial). Current RN license and CPR status required. Strong clinical and communication skills are needed in addition to strong computer skills.
Aide provides patient care and follows plan of care. Must have 75 hour Home Health Aide supervised training course or nurse aide training program. Six months experience as a Nurse Aide strongly preferred.
Current CPR status required. Current driver’s license and auto insurance required along with available transportation to travel to patient residences.
Must be able to meet physical requirements of the position. Contingent positions available – contingent position has no set hours or schedule; works as needed to cover staff absences.
Hospice of Siouxland seeks licensed/certified therapists to provide the following complimentary therapies to patients of Hospice of Siouxland. These will be contracted positions in our tri-state service area. Therapist must have their own pet/equipment to provide therapy services.
