Nurse Practitioner will perform Hospice patient physical assessments and recommend and order treatment interventions within the ARNP scope of practice. Practice can occur at the patient’s home, nursing facility, or other site of patient’s care. The ARNP is a member of the Hospice interdisciplinary team and participates in all aspects of Hospice service delivery. ARNP works in collaboration with the Hospice patient’s attending physician in care of their patient.
Master’s degree in nursing with completion of Iowa board approved advanced practice master’s program or completion of a formal advanced practice education program required. Current NP certification by a national professional certifying body recognized by the IA, NE, and SD Boards of Nursing. Current ARNP license issued by the states of IA, NE and SD Boards of Nursing. Current prescriptive authority through the State of Iowa Board of Pharmacy, and current DEA registration required. Must be registered as a practitioner with the Federal Drug Enforcement Administration. Minimum of one year recent clinical nursing experience required.
Full-time position (30 hours per week) available for a Chaplain to provide spiritual care services which include identification of spiritual needs of patients and families and networking with community pastors to assure spiritual needs are being met. Provides spiritual counseling to patients and families respecting their belief system. Bachelor’s degree in theology, counseling, psychology, or similar field required; master’s degree preferred. Two years of active ministry experience strongly preferred. Excellent interpersonal and counseling skills are required along with the ability to address spiritual care issues and concerns for all religious faiths. Minimum 2 units Clinical Pastoral Education preferred. Basic computer skills/knowledge required.
Social Worker provides a wide variety of social work services which include patient/family counseling, networking of available resources, and aligning them with the appropriate services to provide a continuum of care. Participates in the interdisciplinary team to develop the plan of care, serving as an advocate for the patient and family. Adheres to the principles of Service Care Standards and the philosophy and mission of Hospice of Siouxland in all aspects of job performance.
Requires a bachelor’s degree in social work, psychology or counseling or related experience. Master’s degree with major course work in social work, psychology, or counseling preferred. Requires current state licensure in IA, NE, and SD or willingness to become licensed. Two years of experience as a Licensed Social Worker preferred. Clinical experience of skills relevant to grief and/or loss beneficial. Must be able to communicate effectively with all members of the health care team. Current driver’s license and auto insurance required. Must have available vehicle to travel to patient residences. Must have or be willing to obtain and maintain CPR and Mandatory Reporter Certification. Basic computer knowledge and skills required.
This is a part-time position that is budgeted for 20 hours per week. The nurse auditor monitors and improves the quality of clinical documentation related to the provision of patient services. Utilizes performance improvement methodology and data analysis to assist and guide performance improvement efforts. Assures documentation meets compliance with hospice Medicare Conditions of Participation, CHAP accreditation standards, state licensure and all other regulatory requirements. Conducts audits and analyzes findings and non-findings. Tracks outcome measures and participates in revising or creating policies, procedures, education programs, and standards of practice related to patient care.
Must be a registered nurse maintaining current licensure in IA, NE, or SD as applicable. Prior experience in health care quality improvement and compliance program preferred along with hospice or home health experience. Requires well-developed communication and interpersonal skills to provide effective interface with patients, professional clinical staff and all other related Hospice staff. Demonstrated experience in medical review and chart audits beneficial. Must be detail-orientated and demonstrate general working knowledge of computers and organization specific software (i.e. word, excel, access, power point, internet, EMR Suncoast, etc.) Knowledge of Medicare Conditions of Participation, knowledge of the Community Health Accreditation Program (CHAP) standards or other accrediting agencies standards required. Must maintain a current driver’s license and auto insurance.
Using clinical nursing knowledge and assessment skills, coordinates referrals and admits patients onto Hospice services. Communicates agency services to health care providers, businesses and community organizations. Approximate schedule is four ten-hour days per week with the day off to be determined and would be included in the weekend overnight back up on call schedule.

Must be an RN with current licensure in Iowa, Nebraska, and South Dakota as applicable. Must have at least two years of acute nursing experience plus one year of home care experience. Excellent communication and interpersonal relationship skills are a must!
Schedule for this position will be Monday through Friday from 5 PM to 9 PM. RN will triage phone calls and make nursing visits to patients in our service area. Minimum one year professional nursing experience strongly preferred; critical care nursing experience preferred. Current CPR status required.

Provides nursing care on the weekends to patients by utilizing the nursing process and meeting established hospice standards. Responds to weekend calls for entire geographic service area and implements appropriate interventions based on sound clinical judgment. Part-time schedule is 12-hour shifts on Saturday and Sunday. Additional hours available to make the position full-time if interested.
Must be a registered nurse with a current license in IA, NE and SD as applicable. Minimum of one year of professional nursing experience required. Community health nursing or oncology experience preferred. Must maintain current CPR status.
Provides nursing care to patients by utilizing the nursing process and meeting established hospice standards. Accountable for assigned caseload in collaboration with other interdisciplinary team members. Serves as advocate for patient and families. One year of clinical experience in acute care setting or equivalent experience required (additional experience in hospice, homecare or oncology beneficial). Current RN license required along with basic computer skills, strong communication and interpersonal skills. Need to be CPR and mandatory reporter certified or willing to become certified.
Hospice of Siouxland seeks licensed/certified therapists to provide the following complimentary therapies to patients of Hospice of Siouxland. These will be contracted positions in our tri-state service area. Therapist must have their own pet/equipment to provide therapy services.
